Home & Community Based Services
For most people, an important goal when recovering from an acquired brain injury or an orthopedic injury is to regain functional ability and to increase their independence in their home and community. Our Home and Community Based Programs provide therapies to people in their own homes and communities to help with this goal.
The services are often used to aid individuals transitioning from a more supported living environment to their own home or a family home.
In-Home Rehabilitation and the Community Living Program are offered as Home and Community Based Programs. An appropriate candidate for these programs is an adult or child who is medically stable and would benefit from a home based rehab program.

In – Home Rehabilitation
In-home Rehabilitation allows individuals to receive therapy in their own homes. Therapists come to the individual’s home and provide individualized programs using an interdisciplinary approach. Services are ordered and directed by physicians and are authorized by insurance. Service areas available include Occupational Therapy, Physical Therapy, Speech and Language Pathology, Vocational Counseling, Social Work, Psychology, Recreation Therapy and Case Management.
The Community Living Program
The Community Living Program is designed to support individuals needing minimal assistance to maintain community living. This service can be provided living in an apartment, house or family home. A licensed therapist establishes individualized programs and staff work with consumers and family to achieve supportive independent community living. The average amount of in-home assistance varies from one hour to 12 hours per week. Services available from this program can include assistance in locating a suitable apartment, medication management, medical appointment management, money management, grocery shopping, meal preparation, transportation and home management.
