Post-acute Transitional Living
Our Post-acute rehabilitation services are designed to restore independence and assist with community re-entry. Individualized programs assist people in learning skills needed to return to home life, employment and personal relationships. Programs are designed to provide continuing rehabilitation for people on-site, in their own homes and in the community.
Transitional Living Services are available to persons 16 years or older. Individuals entering the program must be medially stable and in need of intensive rehabilitation.
Our facilities are specially designed to provide a spacious, barrier-free, homelike atmosphere and are located in park like settings close to many community resources.
